Tuesday – Patient Appreciation Day
Celebrating the Heart of Our Mission

Our patients are at the center of everything we do. Today, we celebrate you and the trust you place in our care teams.

What to Expect:
• Providers and care teams will take time to personally thank patients during visits
• Patients and care teams will be celebrated through special appreciation messages (with appropriate consent)
• Join us for the KC CARE Cookout (12:00PM-2:00PM), featuring food, refreshments, and an opportunity for patients, staff, and community members to connect

Wednesday – Health & Wellness Focus Day
Empowering You Through Health Education

We’re highlighting resources and information to help patients take an active role in their health and well-being.

Health Topics Featured:
• Diabetes care and A1c management
• Healthy Beginnings maternal health services

What to Expect:
• Health education resources available at check-in
• Healthy recipe ideas and wellness tips to support your health goals

Thursday – Community Wellness Focus Day
Connecting You With Resources and Support

Healthy communities are built through connection. Today, we’re highlighting the programs, services, and resources available to support the diverse communities we serve.

What to Expect:
• Wellness resource bags with helpful health and community items
• Wellness pledge cards to encourage personal health goals: “This month I will…”
• Information about services and resources available to support your wellness journey
